That particular click is not yet reflected in Google Analytics and with more than 5 niches in that site, it's hard to tell which page that click originated. I'm not so sure about being an Adword error because if you look at lawyer-related niches, they pay really well. Now that's the hint.

These clicks are possible to get in AdSense accounts and not just by newbies that don't know what they are doing.
Mistakes may also happen by Pros when they type Max CPC as $42. instead of $4.2 . Since it is estimated that G takes a 50% cut than the $21. dollar click.
Very high dollar click "mistakes" may happen in Any niche but not only because of mistakes, some niches really pay quite well. You just need to find the right keywords and understand that the thaffic will not be gigantic. Just profitable!

agree. mistake or not, i'm still happy. my average daily adsense earning is just $5. was totally shocked with $21. yesterday, my earnings reached around $28

It is real. My best sites make $10 to $15 per click, though the traffic is not as regular as the sites making $1 to $3 for me

Yes,
The couple I got were for DWI Lawyers. However, they are few and far between and I would say they are because:
1) The person is new to AdWords and made a mistake, they will fast find out its not sustainable to pay so much for a single click
2) They are black hat and have a ton of vouchers and just use those so they can afford to drop more just to be sure their ads are displayed. Still, this will probably dry up in time as well.

Not just blachatters. G was promoting Adwords hard in several countries this last June. So any new user, with coupons, could "pay" for those high CPCs.
Maybe not so careless anymore...
