In off-page SEO, should clients pay only after seeing ranking improvements, or should they pay based on the work done (e.g., guest posts, backlinks, and link insertions)? Since SEO results can take time, what’s the best payment approach for fairness to both parties?
 
Would you be securing the Guests post / Link inserts for no money ?

It depends on terms you both can come with.

in a normal world, yes, that's how you should pay (for results only). But since we don't live in a normal world and - more importantly - since SEO sellers rely on google's whims, it would be a mistake for SEO sellers to only get paid for results because when google updates the algo (which apparently is every other month nowadays) you will have to work for free to fix the stuff that google breaks, and I don't think you want this :)

You do not decide the payment term here. Anyone accepting these payment terms will be mediocre in his work. This can do more hard to your websites.
 
Even though most of the sellers won't bring any good (if any) results, paying based on results is not ideal for sellers,
Sellers would need to make sure the overall website (on-page) is in point, which is not part of the deal, should be billed separately,
As well as niche/KW research, so they make sure their links would be enough to make you rank, that's again not part of the deal,
Results take time, up to 3 months for the full effect, not everyone willing to wait for that time to get paid for their work, and neither it's fair,
And lastly, define "results" here, is going from the 5th to the 4th page considered results? any type of improvements? that will depend on the buyer expectations, and often, especially for newbies and non-SEOers, it's damn high, wanting to dominate the niche with a tiny budget,

That could work if you're hiring an SEOer to take care of everything, basically as a webmaster, where SEO is the main traffic source,
You have to pay for everything done & all of the work mentioned above, and have a margin of the delivered results set as guarantees,
But, at this point, why should anyone work for you? he's doing all of the work, and if he knows what's he's doing, he most likely has the budget to make the website and compete on his own, which many do, the only thing holding honest/genuine link sellers from doing so is that mentioned work

You'll struggle to find someone who works only for results based wage. But you can keep a lucrative results based incentive, whereby the actual pay is lesser than what you'd pay if they get you results.
 
It is best to create a hybrid payment structure. Paying for work done (e.g. guest posts or link insertions) is a good basis, but tying a certain portion of the payment to improved rankings can build trust.

I think you can follow a combination method. You can take an amount for getting links from other sites to insert on your clients' site and then after completion of the process, you can charge your fees. hope you goit my point.

If they hire you for a package, you will receive a one-time fee from the beginning.

"Pay after results" still exists? I don't know, I think. If you're handling only off-page SEO, it's better to get paid for the work you've done.
