If the server is behind some firewall, the IP seen outside will be that firewall. Otherwise yes, a dedicated server must have its own dedicated IP.

Both are not same. Some SEO hosts own few servers in few datacenters around the world then they have large pool of IP which is shared among those looking for different hosts. Then there are some SEO hosts who would have a system of owning large pool of premium web hosting packages from where you can rent your space for PBNs. What you need depends upon what is your requirement and how far you can go to camouflage your PBNs.
