I have got a site that I need changed to a new domain. I want a clean, simple and modern design now in HTML5. There is a sql database that needs to be transferred over. This is urgent and needs to be started on today!

You need to be a web design rock star and have php knowledge and experience.

This will be long term work, so please, send portfolio first of sites you have personally designed. I want to work with YOU, the designer/coder directly, NOT with outsourcing companies or individuals.
