Reward surveys

Hello BHW, I would like to add reward surveys to my existing niche website , but I don't know if it would be better as phone app or should I put it together with the website, what would you recommend ?

What are your favorite scripts for such thing ? I found some phone apps for good price but I couldn't find any website scripts.
