Review Filter Script

A couple years ago somebody posted a simple online review filter script.

I've been looking in the archives and can't seem to find it. Does anybody have access to that script?

Basically, on the home page it says "how was your service" then it had a button for "bad" or "good" if you clicked bad, it went to an online email form and if you clicked good, it took you to links to tripadvisor, yelp, etc.

I could really use this for my restaurant.
