Hi, do you repeat the focus keyword in H2 and H3 multiple times?

Example - Focus keyword is low cholesterol diet

H2 - low cholesterol diet plan

H2 - low cholesterol diet foods

H3 - low cholesterol diet recipes

H3 - low cholesterol diet vegeterian

Do you repeat the focus keyword like that?
 
I don't like it cause it looks spammy... even though I used to do it like just like this :D

Just write for the user and don't overstuff your content with the same keyword, you might actually get penalized for overoptimization.
 
These many times is absolutely not recommended!

It looks spammy.

Use Yoast SEO plugin, it gives you a good estimate of how many times a key phrase has been used and whether it should be used in h2 and h3 or not depending upon the length of the article. See the screenshot attached -

Think of it like writing a book.

H1 is the title

H2 is a chapter

H3 is the sub-chapter. The chapter within the chapter.

H2 is a chapter

I learned this from an article that i read many many years ago.

Maybe im wrong in how im looking at it, but it would make sense, and thats how i use them.

don't ... it will not help you this way.

best way to figure out the best approach: see how the top 3 sites for your keyword have done it. see what they have included in their H tags and how they have addressed user's intention. do the same but better!

use this for each competitor and see how many times they have used the keyword in each H tag. you need to get close to those numbers.

Google has evolved in terms of search intent and relevance within the article body
Just keep it natural and use a table of contents plugin

Yep nowadays google serp is not stable so have to look out for that xD

Hmm good idea. but the first 5 sites are just authority sites. they are not at all optimized yet ranking due to the power of the domain.

It looks spammy and it makes your UX worst that would ultimately result in higher bounce rate.

Yes, I mostly do it like this.
