You can manually remove a follower yourself. (That’s in case you didn’t know)

I don't know about free apps that can get rid of ghost followers slowly to avoid bans on your account. But there is a cheap one called Cleaner that is working quite good. For $5 you can remove as much ghost followers as you want. For android and ios. You can select inactives, ghosts or non profile photo followers. In both versions you can set how many actions you want it takes every hour. The IOS version is bit better cause you can even set the time between each action and you can add a random delay on it to make it looks even more natural.
The engagement is not gonna decrease cause they are ghost followers which means they are already not engaging with the content. Removing them you get the same engagement in amount but the engagement rate in percentage is improving which is giving better score to the account. About the bans you can avoid them setting the maximum batch actions at 50 per hour or less to be safe.

I've never seen a ghost account following strategy that has increased follower growth. I've seen it increase engagement (since you have a lower follower base), but I have the gut feeling that it halts follower growth in its tracks. I've seen it happen to a few pages already.

As @fxmanaged says it will lower the followers' growth and even if the engagement score will rise due to inactivity of ghosts, the blocks will the there, I've experienced it already, I knwo what I am talking about. I am recommending removing the ghosts very very slow. A couple of 10s per day, maybe.

I'm also interested in removing ghost/fake followers. How can these apps determine which accounts are "ghost"? I'm curious what's the criteria they use to make sure they won't remove real people accounts who're just inactive from time to time.

Can I ask why you would want to remove followers? Having a larger follower count not "following" provides what is called "social proof".
In my opinion I would keep any and all followers as long as they are not correlated to anything that is "not approved" by Instagram. If they are not liking or engaging on your content so be it just keep creating and new more engaged followers will come.

The only valid goal to remove "ghost" or fake followers in my opinion is to improve your follower growth. If higher engagement would always result in greater reach to non-followers, then it would also increase the number of followers actually gained. Unfortunately, from what I've seen, this is not the case. I have seen pages remove a large number of inactive/ghost/fake followers and increase both total engagement and obviously engagement rate. Unfortunately, this has not resulted in a corresponding increase of new followers. Quite the contrary, I've seen their new follower growth be completely flat or minuscule at best.

Every account and case is different however with that said removing followers generally will not have effect on much of anything other than hurting social proof , overall the goal is to not worry about current followers but to produce quality content and reach out for new growth.

I hear you, but that's not the case if someone injects a bunch of fake followers at once in your account. It'll screw up the account, regardless of the "social proof" benefit.

Manually removal
You can remove fake followers by yourself. All you need to is to click on the "Followers" tab in your profile. Check every single follower if it real or not. Click on remove or either block & unblock them. Don't do it too quick and don't remove too many followers in one session. Since Instagram new updates they're blocking people from doing actions if they see quick or too many actions.
Note: This method is not good to remove ghost followers.

There’re many apps that could help you to remove fake followers. From my experience they're inaccurate and missing sometime and could remove real followers. I won't really recommend for you to do it but it's an option.

I recommend you take your time and remove them manually. With that you are avoiding many useless Instagram tools that promise many things but do nothing.
