Can anyone recommend a free database as a service for a MySQL database? I'm looking for something similar to mlab but for MySQL.

I'm looking for a managed DB service that supports MySQL and has a free/sandbox option. Yes, I could spin-up a VPS running MySQL but I'm trying to avoid that because it would be an additional cost even if it's $5 monthly.

How about firebase? Not sure what they use inside though.

Unfortunately they do not.
I did find a service called sqlclusters.com that offers hosting plans for $7 which seem reasonable.
Firebase use's their own NoSQL db.
Maybe in this case better to contact traditional VPS providers and ask if they can provide you a kind of custom solution?

just mysql? nothing else? then spin mysql as a service, try digital ocean, 15$/month (I'm not affiliated with them in any way).
there is nothing managed that is for free, don't bother looking for it.
