Ranking well on google PK but not google.com

Has anyone a idea why my site ranks page 1 with most keywords on .com.pk but nowhere on .com or other tlds.
Its a english language site and hosted in the Netherlands.

Is it relative to the US? Is the English good? Have you properly configured hreflang tags if you use them?

Also, much higher competition in the .com.

1- What is the TLD of the domain
2- High Competition
3- Did you sign up with google webmaster tools and tried to change the geographic target of the page to USA?
