A few small businesses (dentists, lawyers, contractors, etc.). want me to do adwords for them for a few small to medium sized towns. How much adspend do you think it will take to get decent traffic/leads and compete with other ads in the area?

Also if I wanted to rank a simple site featuring local businesses for a small/med sized town on the first page of google, how long do you think that would take?

Ad spend would mostly depend on how pricy your keyword is, and this is dependent on how many other people are targeting them. The only way to know this is to try running ads yourself. I think Google offers free $100 for new accounts to try the system.

For your SEO question, if your competition is very weak and your site is well optimized for the locality you can rank pretty quickly. But like the case of Google, you can only have an idea when your site goes live and see where it settles. Meanwhile, open the SERP of your target keywords and see what kinds of sites/pages are displayed. Their strength and optimization would determine how easily you can replace them.
 
The more narrow or local you go the more expensive ads would be, for the local cases it is best to try different approach, broad audience and only select city as target region, and totally open audience only select city as target region.

The rates could not be determined beforehand, you will need to test, also make many ad creative to get optimum rates, i would recommend "google performance max ads" with at least 20 ad creatives
