Say I signed up for something at McDonalds through their wireless internet access - when they check for fraud... will it show McDonalds or only the city, state - or what? What information will they see about the IP address?

They will be able to see the IP address and whatever information the registrar the McDonald's is using provides + some IP->location information. (Automated systems will just see location and the hostname of the McDonalds.) If someone was determined they could get to the exact McDonald's if they wanted to--I would not worry about it unless you were doing something that would get law enforcement involved.
