question about hosting cpa join form

I'm an affiliate for dating gold and they have an option where you can host their sign up form on your own server and post the free lead to them . I have all the information from my aff manager and i know about setting up forms but for some reason keep getting an error with it when i try to test it out. They r a gr8 affiliate but def. made it clear that if u can't get it working urself then they can't help you set it up . If anyone else host their join form or knows anything about setting it up i would like some input into what i might be doing wrong. Send me Pm if u can help.
