A proxy is basically a server.
If you have the proxy: 34.54.65.76:8080 it means that a computer (VPS, dedi), which has the IP address 34.54.65.76 has a proxy server running on port 8080.
To set up your own proxies, you first need servers, with different IPs. After you get those, you have to setup a proxy servers, like squid, to actually be able to use those IPs from different locations.
It's not rocket science, actually rocket science might not be rocket science after all lol, but it's not a job for newbies either.
I suggest you to just buy proxies from a proxy provider, it will be easier for you.

What's the difference between regular dedicated proxies, Pokemon Go Proxies, footsite proxies, etc.
They all seem to be dedicated proxies but the markup on Pokemon Go proxies and Footsite proxies is very high.
Is it just a marketing scheme by proxy companies?

The issue with public proxies is that there are services and API's that are dedicated to constantly scan public Proxy lists, so when checking an IP it can be determined to be a proxy or not. For better a reliability and a lower chance of detection, I recommend to purchase or find private proxies. don't take this for a fact because this is what I know from experience and I'm not completely positive if this is how the system works.

You're only limited by the number of IPs assigned to the VPS in that scenario. You can assign an internal listening port to an outgoing IP address through the use of Squid ACLs. It would be better if you had a mix of ranges though.
Example:
10.10.1.1:3128 -> 134.xxx.xxx.xx1
10.10.1.1:3129 -> 134.xxx.xxx.xx2
10.10.1.1:3130 -> 134.xxx.xxx.xx3
