Hi, I need an order form created for a standard html site that's not using a cms.
It is for a product that has five main drop down options (manufacturer), which triggers another drop down with options.
There will be a tick box for two options.
There will also be a tick box to agree to the terms.
Name, company name, address, delivery address, tel number and email address fields are also needed, most of which will be mandatory.
A basic captcha (something like 3+7= ) rather than words is also needed.
On send the form should send an email to the webmaster and a confirmation email to the customer email address. No prices or payment are needed.
If anyone wants to give me an indication of cost and when they could have it ready that would be good.
