No PMTA will be preinstalled you didn't need to buy just the server and ips and your domain name that's it.

Hi :), Please check at BHW marketplace you will find many reputated and cheap server providers with multiple ips.

Yes you need to pay for your server fees every month to your server provider. Also, You need to buy a domain for which you need to pay renewal fees yearly if you want to use it for long term.
 
What Interspire version will you install?
Can you configure an already installed PMTA on a server which is sending all mails to spam? In that case, what's the price?

Hi :) Currently I am installing mailwizz only. Sorry, i can't able to configure an already installed PMTA which is sending spam because there are few reasons for that why its sending spam maybe its ips are being blacklisted or the email content is been flagged in Spamhaus or the list has too many spam traps or bounce emails. There are many reasons why an email go to spam even if it's legitimate.
