no. You asked if it's OK for page title and ALT text (I assume ALT text of the image) to be the same, and I responded YES because they can be the same without problem. It's actually recommended that they're the same for on-page optimization benefits
 
you won't have such a big impact (bad or good) using alt text mainly,
placing the tile as a picture alt text can be good to go, not a red flag, especially for the featured/1st image,
doing it for all the images in a single article sounds like an over optimising

i wouldn't really bother doing such a thing as it will look a little bit spammy and won't have any actual impact in ranking other than google images

It's valid, but not recommended, There is a structure we have to follow for SEO tactics. For alt text you have to show the info for the photo, And for the title you have to focus on content the photo associates to.
