Virus scan does not detect malicious codes in .php files. also, codes can be hidden in .js and images.
From 60 antivirus engines, 0 will report backdoors, hidden links, iframes, upload script, etc.
it must be checked manually, or with regex search going through all files, and you need to know what and where to search.

Yep, definitely go for a manual check. Most of the time you will find some evil stuff.
A theme is the base you build your site on, I'll never understand why someone would risk his business just to save a few bucks. It's not like themes are expensive and if let's say $79 sound like too much money, you maybe should think about looking for a different business.

use tac (theme authenticity checker) plugin, it may help but manual check more recommended.
