Good question.I was searching google today about this as I would love to download some nulled plugins. Google results just all try to scare the crap out of you that it will have a backdoor/send spam and tank rankings. I am not a coder so looking over the files I would have no idea what to look for to ensure is safe. Guess the best you can do is base trust or not on the source you get it from. Unsure, if any virus scanner is designed to find malicious code in wordpress themes or plugins...

Using nulled wordpress themes and plugins is always risky, without going through full code and looking for any additionaly added malicious code which can include backdoors and access to your backend. In case of wordpress themes, which are composed by a big number of PHP files, it can be quite hard and time consuming to find the malicious code. Additionally, most of known hosting providers do not allow the use of nulled themes/plugins and will delete them if they are detected.

firstly not all themes are "nulled" if someone simply bought a theme and share it then that doesn't count as nulled.
Usually nulling is done to bypass the "activation" step for updates, demo installs etc. That includes custom coding and that means there is always a risk.
You can install plugins like Sucuri or Wordfence to do a basic scan.

Best to avoid. Why there are nulled themes? If they have it for traffic good owner will not inject scripts in it, but where is his source, maybe this one do the dirty job and push it forward...
Scanner search for known malware if someone put custom in noone will find it.
I use such custom backdoors years back (kazaa) and build big 5000 botnet.
My advice. Find free solution there are many
