Not getting clicks, any clues??

I have sent thousands of traffic from fbook to my site for a cpa offer but the clicks aren't registering through the affiliate website. (facebook traffic is allowed)
Anyone have a clue why this could be?

The referer link is "index.php" from that same domain/page.. not sure what is going on and my AM is puzzled too. He actually filled out an offer and still no click or convert.

anyone have any ideas??

could it be that my domain is on some "blacklist"??

found out that it is something with the cpa's network tracking system.
