    I've never seen a WordPRess theme that has any functionality like this built in. You might be able to implement a quiz plugin or something of that nature to program some kind of "if x + y then z" functionality. I've seen some pretty crazy custom stuff using formidable, too. You're probably gonna have to hire a programmer. Some custom javascript should be able to handle this sort of thing - but to get it looking as slick as this will take some time & $$.

    Seems like your chosen theme is very rare, mate! Haven't seen one. I hope somebody with programming skills (I presume. LOL) can read this post and surely will help you.

    That's because it's not a WordPress theme.....

    To be honest with the OP, that landing page wouldn't convert particularly well anyway. The only reason Google use it is because they're a huge company with massive margins who can play about a bit.

    At the affiliate level, keep it simple and focus on conversions, rather than making things look pretty.
