I am in need of someone to do some HTML / PHP form work for me. Its a fairly simple form that will be placed on a landing page. The form will need the following input boxes:
Email Address:
Confirm Email:
Choose Password:
Phone Number:
I want this form sent to my email every time someone submits it. I don't want this form to open up Outlook Express when submitted but redirect to another page once emailed. Also if this is possible instead of saying it came from my sites email or whatever you want to call it it would say it came from the email address the client provided so that I can use an auto responder and it will email the client instead of my sites email.
Currently I don't have much cash on me but in return I could provide graphics design / PVA's / blog setup. Please let me know if we can work something out. Need this done TODAY!
