Need some help? Proxy user name and Pw?

Somebullshit on my computer i popping up with one of my proxys and asking for a user name and pw. Before it never asked for that. I have no idea, i get this when i run mozilla on my Proxy profiles. Its a bluebox and the title bar is the IP address. I cant figure out what username and password it needs

not sure abt this..maybe some experts will reply soon i guess. otherwise, just do a search on the bhw..
