Yes, FFAP works, and I highly recommend it. Are you using exclusive proxies? If not, I am willing to bet this is your problem.

You should always stick to using bots/programs - Faster, much more effective and everything is automatic

Is it a good idea to blind add friends in this way to a fan page for a website?
Would this kinda method provide good conversion rates? or is it not better to find people manually through targeted or related groups etc?

It depends on your niche. If you are wanting to just make a random "fun" page, then just add people randomly and invite them to your page.
If you are targeted a niche, you can do it all by hand - You will get good results. But this is still slow. So you could visit fanpages/groups that are specific to your niche (i.e a fitness group, "get muscle quick" group, blah blah) Of course avoiding the fake pages, or ones with FBML (3 step) and then scrape the ID's of the users and add them as friends.
Then there is geo-targeting which you could do by hand

Geo-targeting rules all. If you can get this done automatically, you are in good shape.
