Need help to create rich snippet

I was working for one of my website. Its title, meta description all were good. I reached on the 2nd page within few weeks, working manually. But recently I noticed, google is showing my site title, description differently and which is not look like come of my content. It happened for all of my pages. Don't know how they(google) are selecting this different content.
When i was clicking on the search results its taking me to my site though I haven't made any change on my site recently.

I saw in google webmaster central blog that, creating a rich snippet can sort me out of this problem. But I have no idea how to get rid of this problem. Please give me some advice, how can I create a rich snippet and make those search result as like my page.
Is there anything can be done from webmaster tools?

Google recommends marking up your HTML with microdata although other methods are also accepted. If you go the microdata route, its just a matter of marking up content that you want to appear in SERPs. There is more about how to do that here,

Note that even though you let Google know through markup what content is to appear in search, they can still choose to ignore it.
