1. Google search: W9 form
2. Print it
3. Fill it out
4. Scan it
5. Email/Fax it
How hard is that?

You can try this site: http://faxzero.com/ (Just found it, so can't vouch for it as I have not used it.)
or Google search: free fax, there are numerous sites with free trials and the one above even accepts PayPal.
