OP - you sure you aren't just trying to get people to complete this form for you to generate bogus leads or something? This really doesn't seem like it would or should be challenging for anyone older than 12. Simply google an online form generator, and go to town. Or use wordpress with a forms plugin, etc... I can seriously think of 100 ways to get this done. It seems like you've put no effort into it all. Thus my comment that it reeks of being a VERY simple lead generation attempt on your part.

You can go to CaptainForm and easily create this type of form and many others: contact forms, feedback forms, surveys, etc. Split your form into multiple pages by adding page breaks. There is no limit to how many page breaks you can use. You can: choose your desired pagination style, rearrange pages and move fields. Good luck!
