need help multi script

I followed the directions given to me to put a multiple direction url redirect script on Wordpress. I installed the script, however everytime I go to my site I get an error message. Can anyone give me directions on how to properly install the scripts?

isnt exactly an error message when i put script in and the view site it says coming soon
