Need a WORKING contact form placed on my site (HTML)

I have an HTML page I had designed for me, and I need the contact form on the page to work. Right now it has the form but the form isn't working, and the guy who designed the page for me has been ignoring me for about a week now.

It will probably take you <5min to get it working if you know what you are doing. I will send you a rar of the html page, the css, images, .php send file, etc.
