Need a Wordpress Plugin to do This!

Hi BHW members, I'm looking for a Wordpress plugin that allows me to do this:
it displays a form on the homepage of my blog and the visitor can enter his info (like email, other data...) after clicking the submit button it will take them to my affiliate link.
Can you point me to a plugin that can do this?

I can't poin to to such a plugin (don't even believe it does exist), but I can develop it.

I think this is easy, but I may be wrong.

1) Use an autoresponder.
2) Set the thank you page to redirect to your affiliate page.
