Need a script for email sending

i hardly could script anything, so i need some help if you have some free time. here's something like what i need:


text is in Lithuanian, Top line is senders email, next is the subject, and the last one is the messege itself. then goes the send button and a link back to the website, but it's unnecessery. i would appreciate if someone could help me with this little script.

I did a quick google search for "send email php form example" and came up with quite a few results. If you look here: http://www.w3schools.com/php/php_mail.asp it will give you a really good base to start with and then you can tweak it to your exact needs.

If you're doing all the work manually you might as well send it yourself manually using your email host.

If you can PM me I can provide with various ways to send emails, either php, perl, or linux. Or recommend software to do it with.
