Need A Sales Page and Re-Direct To A URL Designed

I need a sales page designed - I will provide the URL - ( similar to some of the Howie Schwartz sales pages ) which will have the sales info and email capture and direct the surfer after submit button to a site URL where the surfer then decides to buy or not.
As the traffic is coming from my sales page I get commission from the main site..similar to an affiliate setup I suppose..
Need this done asap please..
