Need a landing page with an Aweber Script built into it.

What I need is a landing page built that will capture basic information such as Name, Phone Number, Email, and them answering a simple question. Once they enter all of this information I need them to be taken to the main website. I will need that information to be sent over to the main website. I also need to have all of this information into an Aweber script so that I can access all of this information to be able to market to everyone who puts in this information.

I would like a price to be able to do this asap.
