If you are self-promoting, most likely your direct links will be from new domains and non-authoritative sites. Layering links will help if this case. Also, layering is a sort of link laundering when you milk link juice from massive amount of crappy link via high quality links.

Not all but majority of links would better be a brand name. In a nutshell, brand is the part of the domain before the .com as what big brand usually are: Amazon (amazon.com), Microsoft (microsoft.com), Walmart (walmart.com), etc...
I didn't say exact match anchor links are not necessary. Actually I think they are the links that boost your ranking the most but now you need to cover it up with overwhelming number of brand and URL links to avoid penalties.
You can ensure certain pages rank for certain keyword by just doing proper on-page SEO and boosting the authority of your domain. Example - create a well optimised on-page product page at Amazon. It will most certainly rank with a handful of links if you choose your keyword correctly. Mostly because of good on-page SEO and huge authority if amazon.com domain.

From 1 to 1,000,000. Seriously, there's no one-size-fits-all recipe. First tier of links in this blueprint is about 20-40. For low competition local SEO keywords you can run two cycles per month and you'll kill all the competitors. Not the same for "payday loans".

I don't pour shit from GSA or Scrapebox directly to the money site (unless it's YouTube video or Amazon or eBay product page).
These 20% of link is where your targeted keyword campaign go, it is where your high PR exact match posts go, etc...

I have only one on-page SEO rule that I don't break and it is to have the keyword you'd like to rank for in the target page title (HTML title tag).
