I´ve seen a number of these in comments, usually they post the moment a huge account posts. Then you go to their bio and they have some kind of PPL offer or CPA
Does it really work that well?

Yeah, it can bring some traffic to your page. How well this traffic converts it's only up to your page quality
 
Algo favors comments with the most number of likes
If any verified user comments under such posts, they'll probably stay at the top instead.
 
Comment spam has gotten old but if you have aged accounts and quality automation tools, it's achievable.

Obviously, not easy or cheap as before though.
 
this has been a thing for years, so I guess if they still do it it makes them good money

The only way to know is by doing it / testing it.
Indeed they bring good traffic but good conversion is not guaranteed. It depends on the traffic type, user country, age, connection, urges and many factors. :)
 
everyone has different opinions about this. On the one hand, this is traffic to your page, on the other hand, Instagram sees that these are bots

Hi! This is quite possible, try and check if you can afford it. This is quite likely to attract traffic.
