Mini Custom Site Needed

Need editable wp version of this page, including subpages when they fill out form. form needs aweber opt in....and to trigger next page. Pretty straight forward, need blank WP, and do straght copy, make sure it works..and i will have my graphics guy mod. 50 bucks, as its pretty straight up gig..would like done today if at all possible.
