I always try the "let us know" option quite a few times and if I don't get any emails through that then I resort to responding to an email I have on file.
I did the "let us know" and they emailed me asking for a screen shot of my issue..

Please provide a screenshot that displays the problem you're seeing. This will help us better understand the issue. To take and send us a screenshot on your mobile phone:
Android
1. Press and hold your phone's power and volume down button at the same time for 1-2 seconds.
2. Open an email application on your phone and locate the email Instagram sent you.
3. Tap the reply button.
4. Attach the photo you took of your screen in your email response.
iOS (iPhone)
1. Press and hold the power and home screen button until the screen flashes.
2. Open an email application on your phone and locate the email Instagram sent you.
3. Tap the reply button.
4. Tap and hold in the email body and a box will pop up.
5. Tap the arrow until you see "Insert Photo or Video."
6. Tap "Insert Photo or Video."
7. Tap "Camera Roll."
8. Find the photo you took of your screen and tap to see it bigger.
9. Tap "Choose" at the bottom right.
10. This will attach the file in your email response.

I also replied to a old email asking to take a pic holding a code and I explained I'm having account trouble and they sent me a new email asking to take a pic holding a sign. I'm not sure what to send for the email asking for a screen shot because that will pretty much be admittting my account is disabled (on the need help signing in form I said I was hacked" but hopefully the other email will be helpful
