Will Maxbounty (based in Canada) send the required U.S. tax forms to me or is up to me to report the earnings that i receive from them?

no, you have to claim in on your own.

We do not issue tax forms, or provide earnings information to government bodies so they can send out forms, for any country. You will need to report your own earnings to your own government.
