Making the move to custom software

What major obstacles do companies encounter when implementing custom software?

Not having clear vision of what they want off the rip (resulting in scope creep making projects harder than should be).

Emotional investments from failiure. (Paying a few devs to do something losing money then writing it off as impossible etc. Resulting in taking possible convoluted altnernative paths.

Sometimes it's best to just nuke and recode everything. Ain't nothing worse than firing a dev for not meeting standards, while you pushed them to continue on a branch that was foundationally built on a whim as a prototype that leads to spaghetti code. Then you hire another dev and force them to start on that spaghetti code.
