Looking to fill forms with tool using proxies

Hello,

Looking for any readily available tool that can allow me to fill forms on sites after setup with the data I provide the tool. i.e I'll provide the name/email/phone number and the tool has to fill up forms on a specific site with varied intervals - using a good residential proxy server and with different mobile phone headers.

What should I look at?
