Looking for suggestions for a mobile checklist (App Maybe ?)

I have a feeling the answer is simple enough but I've been looking everywhere and can't find the solution.

I have guys out working for me who inspect things and fill out a checklist. Its literally fifty tick boxes that get a check mark or a cross. At present its all done on paper. I get the reports in the evening then put them together into a pdf and send it out to clients.

What would make life easier is if they could tick off the checklist on a smartphone or tablet and email it on to me. So looking for suggestions on how I could do that....any software or maybe App builders can let me know if an App is the way to do it ?

Cheers the checklist for ios looks like it could be a possibility. I'll load it up later when I have an ios7 device about.

Any Android developers have an idea on how hard it would be to put together a branded app ?
 
I'm assuming there are multiple employees who are all filling out their own checklist and the checklist for each person is the same?

Would a cloud-based spreadsheet work (Drive, or something similar). You could give each person their own sheet in a master spreadsheet and then collating everything into a master document at the end of each day could be automated.

In the Android store you can also Email the developers, so find an app that has most of the features you need, then email the developer to see how much the custom branding would be. I would think they would be willing to do a deal, as adding branding to an existing app shouldn't be too difficult.
 
I've downloaded that checklist app and its very good. Not ideal for this exact purpose but I will definitely use it for prelim work.

The Drive idea sound a runner. I have everyone set up on Google Docs and I've just uploaded the checklist as a word doc into drive. Now I need to work out how to make it easily checkable and/or editable in Drive. Any suggestions gratefully received.
 
Personally I would go with a spreadsheet over a document because it is so much easier to extract and manipulate the data.

With a spreadsheet you can collate the data from all sheets into a master sheet, which would also make it much easier for you to check everyone's activity - the collation would be automatic so your master document would be a realtime view of where everyone was at.

You can also set permissions for each sheet separately, so only the designated person can update their own sheet.

Plus, you can set up notifications if you want, so you will know whenever the spreadsheet has been updated.
