Damn people are lazy these days.

I use Quform. It is fully responsive and quite easy to customize and works 100%, also enters submitted data into a database.

It is a paid form that I purchased. I would post it, but can't post files here yet. PM me and I can send the Quform package to you if you are interested.

The plug-in version I use is not for WordPress, however, you could implement it into a WP site fairly easily. Cheers.
