Looking for a Wordpress Plugin that does the following

I am not sure what to search for specifically so all I can do is describe what I am looking for, hopefully someone can help me out!

I am looking for a plugin that can replicate a functionality similar to that of the Feedback button on the www.majesticseo.com sidebar that shows up after you do a search, that when clicked slides out (I think using JQuery? I am not sure because I am not a designer).

Rep + thanks will be given, open to any alternative suggestions as well!

yes, for sure that is jQuery, but i am not aware of ready to use solution to combine the floating button and the feedback form
