I think it depends on exactly what look and feel or functionality you're looking for.
For example you can change the theme, background, font style, add a header image, etc.

Although Google Forms has its limitations for advanced customization, you may be able to find workarounds using Add-Ons.

I highly recommend Google Sheets. But if you're looking for alternative options, Microsoft Excel Online also works well and has similar protection features.
