affiliatebuzz and affiliate-robot won't even respond after a week. Don't really want to use netflix.

why you dont want netflix ? its better I think just follow their TOS

Clickbank has one. Netflix's is probably the best recognized / trusted and therefore the best

Does using a content locker and redirecting to netflix after a survey has been completed violate the TOS? In other words, I want to content lock a button that says click here to watch full movie and then redirect to movie affiliate site after the survey or whatever is completed.

Netflix's TOS. I doubt it. All's fair in love and war (marketing is war ). Read their terms to be certain though.
