Looking for a Content locker - "Are you human?"

I use Adscend Media now but what I can't stand is they place multiple offers and their gateways look a little confusing. I want to control the offers and want something that is simple and to the point. Basically, "FILL OUT THE OFFER BELOW TO PROVE YOU ARE HUMAN!" type of text and only one offer that I choose.
