Looking for a coder

I am looking for a coder to:

1. Create a web based application with a contact form
2. The contact form should send a default message to a number of addresses with the information provided in the contact form in the template. For example:

The template chosen by me might be:

Hi,

I am X1. You are X2.

Thank you

and the imput fields on the website might be "Your name" "His name" and so the website should send a mail looking like this:

Hi,

I am Steven. You are James.

Thank you

3. The website should create a temp mail when someone sends this form and send the mail from it. The sender will enter his mail on the website while submitting the form and so the temp should be forwarded to this mail when mails are recviced.

So send me a private message, how much would I have to pay you? How long will it take you? What languages would you do it in?
