what do you recommend on hosting something like this? trademark issues? im still a noob and kinda of skeptical into jumping into stuff like this too early. i wont do it, just want to know for info purposes.
should the button go to the offer? or does that input box insert something into the offer? is that even possible? lol

1) Hosting wise, I'd go for offshore if you're hesitant. Of course, covering your ass just a little will go a long way. In regards to copyright infringement, I've learnt from experience that as long as you comply with any DMCA notices that get sent to you, corps really aren't bothered about going after you.
After all, Apple has a million and one patent lawsuits on going, they're not really likely to go after a small fry.
I'd like to point out however, that I am not a legal professional, and therefor am not qualified to act as your legal counsel, and that this landing page is to be used at will.
2) Yes, the button should link to the offer, There's a variable $button_link or $offer_url (can't remember which) which allows you to set the offer url.
3) The input box doesn't insert something into the offer, but it's definitely possible, and is known as pre-popping. However, I've left this feature out as I haven't had time to look at a decent amount of Apple offers, i.e. I have no idea how many have pre-pop functionality, plus they could all use different parameters (data is sent to them via something called HTTP GET which means adding what's known as a parameter i.e. "email" to the end of a url, such as http://offer.url/?email=).
I may add it in the future if there's a call for it, and enough people know how to pre-pop, but for now the form is static and purely designed to increase the "legitimacy" of the offer.
