Back in the 1980s, the mouse cursor was actually straight up and down. The issue was that it looked terrible because of how low the screen resolution was. It looked a lot better if it was turned at an angle. So they turned it until it fit neatly between the pixels. They did it this way because if they had kept it symmetrical, it would have blurred all over the place when you moved it.
In later years, Microsoft attempted to stylize the cursor in a variety of different ways but the problem was that everyone would complain if they changed it even a little. Everyone had gotten used to the tilted, unsymmetrical version.
INTERESTLY, macintosh had a similar issue. For a number of years, they shipped their operating system with a mouse cursor that looked like a little hand pointing... however people complained that it did not look professional. It looked too janky and less serious than the Windows3.1 mouse cursor. By the mid 1990s, both companies just stopped playing with it.
