A large part of the accounts promoting adult/sex are hacked ones, which have already high trust rank and can get away with lots of actions.
As for the meme accounts, if they have a decent history of past activity it can be the same case as above, higher trust rank that allows many actions - compared to accounts without much past activity.
Blocks are usually more intense for API calls, if you use web calls or the mobile app, most of the blocks (and other issues or glitches) should go away.

If you use jarvee than disabling calls from the API should do the job.
